embed issues

Hi. I've tried to embed this form on my shopify site in several different ways and none of them are working. I've pulled the simple embed code and it's pulling a screen grab of my site into the form itself. i've tried pasting the source code into the html field and it's adding weird bullet points. I've tried creating a page template and pasting the code there and that's breaking the styling of my site. help!
