Hi,
IOS does not deal well with javascript embedded forms in some cases. It looks like yours is one of those cases. Try replacing your current embed code with the iFrame embed code:
https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
This should allow for your form to show properly on IOS devices.
If it does not, let us know and we will be happy to have another look.
