Hi Sir,
I want to embed a form to my html file as below link and html

</html>However, when I view this page in iphone(safari), the form will be not normal.
Can you help me? Actually, I want use this form in all mobile devices.
Now I will attached some photos which is not embedded in html file, this is show as well.

Instead of using the script embedding method, kindly use the iframe embedding method

preview(it is no embed to anywhere, only use the joyform)

Kindly add the mobile responsive widget to you form and also enable form responsiveness under your form preferences
