If I understood you correctly you did not receive all data from your form when you submitted your form.
I have inspected your Email Notification and I noticed that it is set to pass data form only 5 fields.
You should add all fields questions and values of those field inside of your Notification.
Or you can simply create new Email Notification and delete old one. When you create new Notification all fields and values of fields will be automatically added inside of your Email Notification.
