Form not sending Leads via Autoresponder

Why are my leads not being sent out?

There are some failed messages only to m***@i-***.com address in account email history. However, the form email settings look good, so this might be an issue with one specific email address.

I have submitted a test entry, and I have successfully receive the form email Autoresponder.
