embedded in it. The form works fine when Enabled, but in Disabled mode, nothing displays. I've tested it in Safari and Chrome.

Hi,
I see you have other JS scripts that are loaded, and I see you also have two scripts that are embedded on that page. It's possible that you are getting a blank message because of a script code conflict.
Could you try using the iFrame embed code instead? That should prevent other external resources to conflict with your forms.
Let us know if that works.
