Fields for the form (Web - Whitepaper - Comparing Video Collaboration Implementation)

    Hello JotForm,

    We have a form called, "Web - Whitepaper - Comparing Video Collaboration Implementation" and the form is being used here.  When we first copied and pasted the code for the form, the fields showed inside the box but now it doesn't show.  It seems to disappear on us... 

    Please help!

    The only problem is, the hint texts on textboxes are not showing. This is most likely due to script conflicts. I suggest to re-embed your form using iframe embed code. This should resolve the problem. https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
