You should re-create the Notification E-mail to be able to include all the fields in the form.
If your notification email lacks some of the existing fields that are in the form, they can also be added while editing the email notification:
Usually, this happen when you update the form and the existing notification email is not updated properly. Creating a new notification will refresh this and will add all the fields.
