How do I connect a PayPal form to my PayPal account?

I cloned a PayPal form template, and attempted to customize it:

Questions:

1. How do I attach the form to my PayPal account?

2. How do I delete the word "Donation" under the payment field?

Click on the payment field, then click on the little wand in the top right of the field.  From there, you should be able to input your PayPal email address to connect the form to your PayPal account:

Changing from collecting donations to selling products will remove the "Donation" label from the field.
