It is not per form. It is the total storage space for your account as a whole.

Do the forms no longer work when limit is reached?

When the limit is reached, your form will be disabled and your account will get suspended unless you upgrade. You need to wait for 1 month when the submission counter resets back to 0. However, if it is related to the storage space, you need to delete the uploaded attached files.

What happens when you complete a form/account that has reached its upload limit?

When they try to access your form, it will show a screen that says "Form Over Quota".
