Here is the step on how to achieve this.

#1 Add the Iframe widget on the main form where you want to embed the intended form

    link: http://widgets.jotform.com/widget/iframe_embed

#2 Get iframe embed code of the intended form using the Publish button

    user guide: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

#3 The iframe embed code you got on step #2 should be added to the iframe widget you added in step #1

Hope this help. Let us know if this did not work for you.

    Hi thanks for the reply,
The scrolling gets stuck on an I pad?
When you then set the page to open up in a new window you can't return to
    complete the form.
Is there a way around this?

I can't seem to replicate the issue you are having. I checked your form (https://form.jotform.co/60452817094861) in an iPad emulator and I didn't notice the scrolling getting stuck. Furthermore, clicking the link to fill the direct debit form lead me to a new tab, so I can easily click on the previous tab to return to the original form.

Hi it may work on an emulator but it doesn't work on the iPad. You can't
scroll further than what you can see in the photo.
    Please see attached.

We apologize for the inconvenience. Unfortunately I do not have an iPad to test the issue so I am using an emulator. It also seems that I am unable to see the screenshot that you have attached. Please follow this guide to attach a screenshot in the forum: How to add screenshots/images to questions to the support forum?

In the meantime, perhaps you can clear your form cache to see if that fixes the issue. Here's how: How to Clear your Form Cache
