How to use 5 star rating survey tool to collect ratings for product/url?

im looking for a way to use jotforms 5 stars rating tool to collect ratings for product/url on our website. avarage value for product and comments from clients should be displayed under the product. we will ensure that code is copied to all our subpages.

You can use the Star Rating tool to rate each product. You can fully customize it by putting a sub-label and editing the label. You can also set a default value in the star rating tool if you like.

On the screenshot below, I've used 3 star rating form fields with a label of products 1, 2 and 3.

hi,
it might be difficult if you have more than 100000 products. we would
like to copy code on sub pages where subpage represnts product. is it
possible to rate url? or even better, part of url? we sell vacations, so
we don't need to differentiate hotel by dates.

Will you embed the form on each webpage URL? If that is the case, then you can use the Get Form Page URL: https://widgets.jotform.com/widget/get_form_page_url

This widget will provide you the URL of the page where the form is embedded, so in that way you will know the rating for each URL.

yes, thats the idea, embeded form at get page url, but we need to show
other users on url the previous ratings and comments from other clients and

Follow these steps after you include the Get Page URL widget: https://www.jotform.com/help/252-How-to-Add-a-Widget-to-your-Form 

1) Include a Iframe Embed widget in your form: http://widgets.jotform.com/widget/iframe_embed 

2) Create a Grid Report, please see the guide: https://www.jotform.com/help/105-How-to-create-a-Grid-Listing-report 

3) Embed the URL of your Grid Report into the Iframe Embed widget:

This is how it will display: https://form.jotform.co/60906019420852

You can submit my form from any of the two webpages and score them differently, you will see the new entry will be updated in the embedded report:

hi,
yes, i have used this before, but issue is that client wants to see
rating for page one on url of page one only and overall rating too. same
for page two on url of page two. we can see all ratings on all pages now.

Unfortunately in that case I believe you need a different form for each page. I will still try to find a workaround that suits your needs on a more simple base. I will contact you as soon as I have more details.
