    Hello... My problem is that when someone submits input in my OHS 30-in-30 form, the email response forwarded to me shows the questions from another different form of mine. I have disabled all my other forms but I still get the reply you see below. I added a request to get a pdf version which is correct but it is still attached to a reply format to another of my forms.
    Does this help? I'd like to get the answers in email form in addition to pdf because it will be easier to collate.

    It sounds like the email notification template got outdated on your new form, as it was created when the form looked differently. To refresh the email notification on your form, please try removing the old one first, and then setting up a new one instead:


    You can do this by going to Emails, clicking on the Delete button of your existing notification, and then on Create New Email to create a new one:
