    Why are ALL my forms showing as "FORM NOT FOUND. THIS FORM IS DISABLED"???

    It appears that your account was auto-suspended by our phishing detector, which is why your forms are disabled. Please note that collecting credit card information is not allowed in jotform, and it is a violation of jotform's terms.

    To lift the suspension, I would suggest you delete these fields from your two forms http://www.jotform.us/form/60937805169162 and http://www.jotform.us/form/60557489662166.

    If you want to collect payment from users, please use one of our payment integrations such as Stripe or Authorize.net.
