    Is there a question form that allows for yes or no and for additional comments.

    I want to ask a condition questions (good, fair, damage), and I want respondent to be able to add comments on the item listed.  

    Is this possible?

    You can add different field types in each questions, example you can use the radio button or drop down field for the "Yes" and "No" options, then add a textarea field for the comments.



    If you are using the Matrix field, unfortunately it will only allow you to have one type of input. It cannot be a mixed of them.
