Unable to submit the form using Wordpress Embed

I created a form: https://form.jotformeu.com/61030227217341

It works fine directly, but after embedding it in WordPress based website: http://ukc.viko.lt/studijos/internetine-apklausa/

It doesn't work after pressing submit button, submit button become "please wait..." and nothing more.

It is possible that there are script conflicts between the page and the form. Please try to re-embed the form using the Iframe method.
