Our template stopped working on our website... It still works for the customer, but we are not getting their submission emails... I have tried reloading the source code on the site, to reset it, but I cant figure out where it is catching... I was able to pull all the submissions and forward them to the proper individuals. but its not the solution.

The notification for that form is not well setup, the Sender E-mail is missing, and the recipient emails need to be separated by a comma and space, I have fixed it for you now:
