    I was able to replicate your issue when I visited your webpage. However, if you go to the direct form, the issue is not happening.

    Can you please try to re-embed the form using the iframe method. This will prevent script conflicts between the form and the webpage.

    Can you please try to re-embed the iframe code without the script element.

    The iframe code is red box and the script code is in the green box. Please use the iframe code only.
