I checked our logs and indeed, the emails notifications for your form are failing to at least one address. It looks like "ary****@wa*****.edu was rejecting emails as spam. When enough were rejected, it resulted in your address being placed on our bounce list to prevented further rejected emails.
I removed your address from the list and notifications should attempt to send once again. For more information on preventing this from happening again. Please see this guide:

If any other addresses are not receiving the notifications, you can remove the from our bounce list on your end:
