Our form is not loading correctly

    Form is not loading with background and all the hidden/conditional fields are showing???? Need help asap.

    The form is working fine upon checking it to my end:

    Based on your screenshot, it seems that the form scripts and CSS are not working. If you are connecting to a private network, make sure to check and allow connections through Jotform. 

    If the form is embedded, I would suggest using our iframe method: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.
