I created a form and published it. Everything looked good and had all of the fields that I wanted. I even shared it with someone and they said it all looked great. Now it is blank with only the title and I've received 2 blank submissionsPage URL:

I have checked the link to the form you have provided and it appears to be fine on my end. If you are still having issues on your view of the form, please do let us know.

Sorry for inconvenience this may caused you. I have checked form revision history of your form and I saw that yesterday all fields were removed from your form.
You can revert your form to previous version (Jun 7). and your data should be restored. However I think that you will lose new data, because you have added new fields. So I would suggest you to download your existing submissions and then to revert your form, to restore missing submitted data.
