I used a template to create a form, but the link is disabled.

    I can pull it up in my saved forms, but when I email the link, it says it is disabled.

    Your form is asking for credit card details, such as depicted in the following image:

    Collection of credit card or banking details is considered a phishing activity and is strictly forbidden by our terms:

    The only way to receive payments through your forms is by using one of our existing payment tools, which integrate in a fully PCI DSS compliant way with the payment processor of that tool:

    In order to have your account reactivated, please make sure to remove the currently existing credit card related form fields from your form.     Ok, thank you.  I removed the payment request.  How soon will my account be reactivated?
