I have a desktop website and I would like to make a mobile version that looks the same. I want the image on the left to be removed or placed at the top, and then the required fields section to be centered in the screen.

I appreciate the help! I did the first step successfully, and I grabbed the iFrame embed code. My next question is, what exactly do I do with the iFrame embed code? I have it copied but I don't know where to place it. From what I understand, there is one more step so that when I open the website on my phone, it'll be mobile responsive.

Paste the code into your web page where you would like to display it. Usually somewhere in the body section of your webpage and within those tags.  I see you have already done so.  The form itself does look to be mobile responsive.
