Answered by EltonCrisI removed your email in the bounce list. You should receive emails by now.
Here's the bounced reason:
rbse@yahoo.com is on the bounce list
Reason(s) : smtp; 554 5.7.9 Message not accepted for policy reasons.
To prevent this from happening again, we suggest to add JotForm noreply emails to your yahoo contact list or sender's list so yahoo will not block any future emails coming from JotForm.
You can also whitelist JotForm mail IPs and domains, https://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses

I don't know how to white-list in yahoo... if that matters, because my other form works fine...

The problem here is the "Sender Email" for the notification. It is set to r**e@yahoo.com. I believe it is related to Yahoo DMARC Policy which is why your email address is always in the bounce list. Please change it to (noreply@jotoform.com) or (noreply@formresponse.com). Here's how:
1. Click the "Emails" button in the top menu.
2. Select the notification and then click the "Edit" button.
3. Go to the "Advanced" tab and change the "Sender Email" by clicking the drop down field. Please change it to (noreply@jotoform.com) or (noreply@formresponse.com).
Since your email address is in the bounce list, I removed it again.
