hi
i added my form to my page in Muse and it works perfectly on a desktop mode, I mean 1200px width.
the problem beging in the Breaking-Points, 768px for tablet and 480px for mobile.
the form is there but not display ion the web.
please help :)

Can you please provide us with the URL of the website where you embedded your form?
You can always try to embed your form by using iFrame method and make your form responsive before you embed.

I have the same problem in Adobe Muse. When I preview/publish I can see the form, but if I reduce the screen size the form "disappears". Then, even if I maximize the screen again, I cannot see the form. Openning on smaller screens (mobile) opens without it (blank).
FYI, I embedded the code on Muse and the mobile responsive IS ticked on the settings.
Have you figure out what is the problem?

If the form disappears when the screen is resized, then it's more likely an issue with the page, and not the form embedded in it. But if in case you are using the javascript embed code in your form, can you try embedding using the iframe code?
