When I choose the option to edit the form and click on the email icon,i do not get the option to set up notification email, just auto responder. Where do I find a way to add notification email info?

I have checked your from and indeed you currently have only one email auto-responder, but there you can set up an email notification by clicking on the button "Create New Email".
