How to "Add Another" Repeating Line to Bulk Order

Hello, trying to create an apparel order entry form, with a set of 4 different entries (item drop-down, size drop-down, quantity, and personalization text). All the templates I saw doing similar didn't allow for ordering the same item multiple times in differing sizes, customizations. So what I need is a table-like entry method whereby the user adds a row as needed, with no limitations. I've seen this kind of functionality elsewhere, using an ADD LINE button that simply gives the user the same set of entry options each line. A repeating entry system within single form.

I think you're looking for a configurable list widget which has dynamic fields functionality where it allows you to duplicate the define fields as many as you like.
