I've followed the instructions very carefully. Email notifications are not being sent to me at email@example.com. They used to be sent and for some reason, they are no longer being sent. Likewise, test messages are not being sent to my email address. I must constantly look at the INBOX for new form submissions. This is a real nuisance. I need your help.

There were failed logs for the given email address, but your most recent submissions are shown as sent. Please check your spam folder if nothing is in your inbox. The following are your recent submissions:
Your last form ID is http://www.jotform.us/form/43145685969168. Based on your form settings, you are using your personal email as the sender email of the form. I would suggest switching it back to firstname.lastname@example.org and test it. If you want to keep your own email as the sender, please use the SMTP method instead of the current verification method.
