JotForm is a free online form builder which helps you create online forms without writing a single line of code. No sign-up required.
We believe that if one user has a question, there could be more users who may have the same question. This is why many of our support forum threads are public and available to be searched and viewed. If you’d like help immediately, feel free to search for a similar question, or submit your question or concern.
Are you referring to your form name/page title in the browser tab? If yes, since the form is embedded, you can't see it there. You'll only see the Facebook tab/app name. If not, please share us with us your Facebook page and let us know what name are you referring to. A screenshot would be helpful.

If this does not resolve the issue you were having, please describe the exact steps we need to take, and provide the link(s) we need to open in order to see the issue, and we will be happy to help. Thank you.
