Hi there,
I have 2 charges on my American Express stating JOTFORM INC. SAN FRAN
1. on the 16th June - 13.84
2. on the 20th June - 219.01
Please can I get a receipt for both charges sent to my email address.

I am not able to find any accounts registered with this email address. Using the IP address, I see that there is an account Maxinefester which was upgraded to Bronze monthly on Jun 16, 2016 and upgraded again to Silver Yearly on Jun 20, 2016. But the account was changed again to Silver monthly plan on the same date.
You may check the invoices in the billing section of the account settings.
