I am not receiving the entry form along the payment notification. This has happened before and it was because my email address somehow got on the bounce list, but that is not the case this time. Not sure what is going on.
Please advise

It seems that the reason is the same in this case. Your email address ended up in our bounce list.
It has been removed from the bounce list now. The email delivery should be restored.
If you updated the email setting to use only our standard noreply@jotform.com sender it should fix the issue completely.
