I believe you are referring to this form (https://www.jotform.us/form/62095459557165).
You are getting this error message because the Unique Submission option is set to "Check cookies and IP". The form is on Strict Check mode. It means that the user that is using the same browser and same IP can only submit once. This option can be changed in the Preferences menu. You can set it to "No Check" to disable the Unique Submissions function.
Click the Preferences in the Form Builder.
Go to Advanced tab and you'll see the Unique Submission option.
I was able to submit the 1st test submission in your form. However, when I try completing your form again, I got the same error.
