Adding payment autorization to existing form

Hello-Third time's a charm. Can I add the Payment Authorization form to an existing form?

Can I add the Authorize Payment template to an existing form or do I have to create a new form and start with the Authorize Payment template. If I use the Authorize Payment template can I customize it or will that violate terms & conditions.
