I am not able to submit requests for the form.

I did a test submission on your direct form and I didn't encounter any issue. If you are having problem submitting the form, it is because you have set a Unique Question for the "Pastor" field.
If you entered the same value as before then the submission will not proceed. You'll get an error stating that only 1 submission is allowed. Here's a guide on how to remove this function:
1. Click the Preferences button in the toolbar.
2. Go to Advanced tab and change the Unique Question to "No Check".
