Conditions get messed up

I have an invitation template that gets filled based on when our member selects their name. When I add new members and order them alphabetically, you'll see how it got all messed up. Is this a result of changing them, reordering them or something else. Help! This is a pain to fix!

I have checked conditions of the form you provided and they look correct now. 

It seems that you were able to resolve your issue. Did you have to redo your conditions? 

I was also not able to replicate mentioned issue on my cloned form. I have added new members and change the order of existing members (inside of dropdown field), but conditions were not changed. Each time data for member that is selected is shown on the form, like it is set inside of conditions. 

If you are still experiencing same issue, please provide us steps how to replicate that issue.
