Revisions on are reverting to old version

Hello, on this form:

It keeps reverting to old versions- it has done this twice (each time about 2 hours worth of work lost- the form has extensive conditional statements). You can see all the activity in the account activity log, but the revision history for the form show's nothing.

also tried clearing the jot form cache. I need the form reverted to it's state from Friday, August 12th at around 5 PM EST.

Please expedite looking into this- this is a form used everyday. Thanks, Chris Driscoll

As I check the revision history of the form, I see that there are some modifications being made to the form. 

I also notice that some of the forms are being shared with your sub-account users. Could you check if any of them are making the changes to the form? I see that there are two versions on August 12 that you may be looking for. If you have not reverted the form to the required version, you may revert it from the Form revision history.
