Missing text and agreement information when form is printed/submitted.

How can I ensure that the information on my form is displayed when the form is submitted? I have three fields - participation, photography and counseling agreement, along with the accompanying agreement text - that the form-completer either clicks to agree with or not agree with. When the form is submitted, the agreement text of these fields doesn't display. I need it to display on this submitted form. Can anyone help?
