Here is my form.  Scroll down to the credit card matrix.

I am printing out and needing blank areas to write.

Before we proceed with your request, please allow me to inform you that collecting Credit Card information and login credentials (username and password) is strictly prohibited in Jotform. This is considered a violation of Jotform's terms, and it may lead to your forms being disabled and your account being auto-suspended by our phishing detector.

I would like to ask you to delete the fields in all your forms that violate our terms.

Once we confirm that these type of fields in all your forms have been deleted, we can proceed with assisting you with your concern.

If you want to collect credit card information without charging them, you will need to use one of the available payment integrations in Jotform such as Authorize.net, Stripe or Paypal. Here's a guide on How to Enable Payment Authorization.

I will wait for your response.
    Regards.
