I have a form that is used for inquiries to our club and we have three required fields within the form. Name, Phone and email address are all required. We recently received an inquiry without two of these fields completed. Today, I just tested the form and it did not accept my submission until all three required fields contained information.
It is a mystery to me how a non-member can submit an inquiry without the required contact information. I wonder how this happened?

To my understanding I see you're having an issue with receiving blank answers for required fields that were submitted via your form.
There are a number of reasons this could happen but the majority of this problem tends to occur with spammers or users that are filling in your form with an obsolete browser and computer.
The "requirement" feature needs javascript to function properly so when that's disabled or not kept up to date with the latest version problems like this can happen and it would be ignored.
Some ways you can combat this problem would be to add a captcha field and also better yet add our NoScript widget to disable your form if the user is detected as having issues with javascript.
If this continues to be an issue though then please feel free to let us know and we'll gladly investigate it further if needed.

Well, I tried it myself with a current browser and it did not fill in 2 of the 3 required fields. Initially, it would not allow me to submit until the fields were filled so that part is working well. I think this form is not configured properly.

Would you mind telling us the name of the form or providing us with a link to the correct form you are having this problem on so we can investigate it further?

The form is titled "Naked Zozobra" . Zozobra is a festival in NM all about burning Old Man Gloom.
I must have configured the form incorrectly. Take a look.

The embedded form https://www.jotform.us/form/61947473562162 in your website must be having script conflict with the other scripts existing on the website. This could be causing the required field validation to misbehave.
I suggest you try first re-embedding your form on the website using instead its iframe embed publish code
User guide: Getting-the-form-iFrame-code
Make sure to clean/remove first any previous script code of the form before adding the iframe code.
Then observer later on if issue is still happening. Let us know if it does.
