    Adding a hint text pop-up to the field

    i would like to provide the users with hints to help organize their answering questions

    so i placed a hint icon and made a url link to this

    would a light box be a better way ?? so that it pops up in the form itself....

    the code below is a bit much for me

    To use a button, just simply use the lightbox class from your lightbox embed code on your buttons code. Example:

    If you would like to add some hint text, you might consider trying a Hover Text option of the form field.

    It will display when the mouse hovers over the field.

    If you are referring to something else, please provide us with a bit more information on your inquiry.

    It is designed for text, but since you can add HTML tags it might be possible to inject the images.

    i teaked what you gave me and it is just great
    our interaction is typical of what i have come to expect from jot form, for
    that reason i am not sure what is going on with the questions as listed
    below....it has been a few weeks and several requests to get a reply on this
    please advise on the below
    the file attached i get as a message fu on autoresponse emails

    has unique field name for pgy and attending name
    and they are captured in this condition re change thanks you page so as to
    pre-populate this form
    date surgery, attending name don’t populate as the other fields do
    these fields are designated as hidden but show up anyway

    Attending e-mail *
    users start by completing the form below

    prepopulate certain fields
    users are then routed to the form below

    which has been pre-filled as below shown
    but fields designated as hidden are revealed
    how can i keep the hidden fields hiddem
    / also format of date field as prepopulated is incorrect

    #month_160, #day_160, #year_160 original
    please advise
    i checkk spam and trash and dont seem to see email for program director as
    but the one for resident comes through
