When I paste the source code into my site the radio and check boxes are not there?

    I have tried numerous thing to get them to show..Any suggestions .. In the screen shot the top question should have radio buttons next to the yes or no tab

    I didn't encounter this issue when I tested the source code on a blank HTML page. The radio buttons are showing properly.

    It is possible that there are script conflicts between the webpage and the form using the source code. Please try re-embedding the form using the iFrame method. Here's a guide: https://www.jotform.com/help/148-Getting-the-form-iFrame-code
