Can I create a two part performance review.

    Would like to have the employee fill-in Part Afor 10 Questions.

    Then have the half of the performance review form filled form forwarded to the supervisor to filling the other half.

     

    Ie:- Staff Comments followed by Manager comments before printing combined form and e-mailing both parties.

    Yes, this is achievable with our forms, and it is most commonly referred to in our forum as "approval workflow". It is usually achieved by setting up an email notification for the supervisor, which will contain an Edit Submission link - so your supervisor can edit the initial submission.

    The form itself should be split into two areas by using the Form Collapse fields. The form will have two sections - one that visible, and should be submitted by your user, and one that is hidden/collapsed, and should be filled by the supervisor.

    We then use conditional logic with a show condition, and show the supervisor's section to the supervisor once he enters a correct "access key":
