JotForm footer keeps on repeating itself

Hi,

My name is Dean Hall (deanohall@gmail.com), I started my free account and filled out my first form.  Then uploaded it to my site below.

You'll notice that at the bottom of the form (submit button), just below "Powered By Jot Form" is being repeated many many times.  Do you know what could be the problem?

I checked the website you provided but I wasn't able to locate the JotForm form on it. Please provide the URL of the webpage where the form is embedded so that we can further investigate.

Also, please try to re-embed the form using the iFrame method. This will prevent script conflicts between the form and the webpage. Here's a guide: Getting-the-form-iFrame-code
