I cloned a form from the main forms drop down menu and the original name of the form appears when the cloned form is opened in a new screen?

Could you please provide to us the form ID that you're cloning and form ID that is the actual clone so we can investigate this since I'm not sure what actually happened?

I did that and the old title (8th Grade Washington DC) is still coming up.In addition, when I either open preview the form or open it in a new browser, the form is already pre-filled with test answers I used previously. I dont understand?

I fixed the title and I was able to remove the responses from appearing on the first page of the form, but they appear to still be pre-loaded on the other pages. I dont know how to delete the radio buttons and non text answers. Its very strange.

I am glad to hear that you were able to change the page title of your form.
