Is there a way to restrict who completes the survey via a list so only those attending will have access?  

heir name for their files as proof of earning the education credits. Certificate is in the form of a template with the name being a merge field.

 

You can use the Show/Hide Field condition wherein if the user entered their name in the field, the condition will activate. Here's a guide:

1. Insert a normal text box in the form.

2. Click the "Conditions" button in the toolbar.

3. Create a Show/Hide Field condition. Set a condition wherein if "John Smith" is entered in the "Student Name", the other fields will show up including the Submit button.
