We have now found a new online form provider, and would like to stop our payment to JotForm.
Can you please change our account to a normal account, and confirm that the automatic payment has stopped. ?

I see you're on Premium yearly plan that ends on July 2017. Are you sure you want to cancel that plan?
To downgrade your subscription please you can go to Billing page:
And click on Downgrade.
Also, since you are on PayPal's subscription please follow this guide on how to Cancel PayPal subscription so you avoid re-subscribing.
