    You can convert an online web form to a fillable PDF in the My Forms page. You just have to select the form and then click the "More" button. After that, you need to click the "Create PDF" option.

    A PDF file will be downloaded to your computer. This PDF file is a fillable form. You can now upload that fillable PDF form in your web server. Once the user click the link, they will be redirected to that fillable PDF or it will be downloaded to their computer.
