How can I integrate the jotform submission form to constant contact

Hello I created a custom form with jotform and add it to my website but I need the name and email to be add to constant contact but when add click integration does not give the options of the name of email that was created, how can i do that

Can you please retry again. Try also refreshing your browser first and relogin to JotForm.
