If I understood you correctly, you want to embed your contact form to "Contact Page" in your website. To do it, you need to copy embed code of your form in JotForm, and then, paste it to your webpage source code.
Please follow the steps below.
1. Open your "Form Builder" and click "Setup&Embed" tab. After that, click the "Publish" button.
2. Click the "Embed" tab in the Publish page, and click the "Copy Code" button.
After that, you need to paste this code to your webpage source code as I mentioned above.
You can also use the Embed Options dropdown menu if you need to customize your form code to fit your own web site. You can find detailed information about "Embed Options" in the following link.
