I am receiving email notifications for this form: http://dynamitesports.com/contact-us/
But I am NOT for this form: http://dynamitesports.com/seminar-request-form/
As of yesterday morning both were working. I didn't change anything... can you please let me know what's going on?

I have checked your form email history, and according to the logs, emails from both form 80452507598 and form 10853824252 are being sent successfully:

The only emails I see there were likely not to be accepted by the recipients, is this test email, because it was missing a Sender Name:

Other than that one, your emails appear to have been delivered successfully from our system.
You are basing the Sender Name on the entry being made in your "Your Name:" field:
If you want to ensure that your form always has Sender Name correctly filled, you may want to mark that field (Your Name:) as required on the form.
Other than that, your emails appear to have been configured correctly, and the logs show these emails as being delivered successfully.
I have also made a test submission on your second form, and I see that the following email was sent from our system:

Please check whether your recipients have received this email, and make sure you/they check the Spam / Bulk / Trash folders as well.

Hello,
We are having email issues recently. JotForm emails are falling into spam folders of some of the email providers. Could you please check your email spam box? If JotForm emails are there, could you please select all of them and mark them as not spam.
